Which Specialties Could See New ASC Opportunities in 2026?

Published: September 17, 2026

CMS expanded the Ambulatory Surgical Center Covered Procedures List for 2026, adding 560 procedures.

The changes include 289 procedures added under revised ASC coverage criteria and another 271 codes added following changes to Medicare’s Inpatient Only List.

For some ASCs, this could create opportunities to expand appropriate case volume.

But the opportunity isn’t the same for every specialty or every facility.

The real question is which procedures fit your physicians and case mix, what your payers will cover, and whether your ASC is financially and operationally ready to perform them.

Which Specialties Should Take a Closer Look?

Orthopedics & Spine

Musculoskeletal procedures are an important area to review following the 2026 changes.

Orthopedic and spine ASCs should compare the updated procedure list against their surgeons’ existing case mix and evaluate which appropriate cases may potentially shift to the ASC setting.

Before expanding volume, review payer coverage, contracted reimbursement, authorization requirements, implant and supply costs, coding, and credentialing.

Cardiology

Cardiovascular ASCs should also review the 2026 changes against the procedures their physicians currently perform.

The opportunity isn’t simply identifying a procedure that can be performed in an ASC. Facilities should evaluate commercial payer coverage, authorization requirements, reimbursement, device and supply costs, credentialing, and clinical capabilities before moving additional case volume.

Vascular

Vascular-focused ASCs should compare the updated ASC list against their current physician case mix and payer contracts.

Before adding or shifting cases, evaluate payer-specific coverage, prior authorization, reimbursement, procedure costs, documentation, coding, and billing requirements.

Pain Management & Other Surgical Specialties

The 2026 ASC expansion reaches beyond a single specialty.

Pain management and other specialty ASCs should review the updated list against procedures their physicians currently perform while paying close attention to payer coverage, medical necessity, authorization, documentation, and reimbursement requirements.

Medicare Coverage Is Only Step One

Finding a relevant procedure on Medicare’s ASC Covered Procedures List doesn’t answer every revenue question.

Before adding new case volume, ASCs should evaluate:

  • Commercial payer coverage
  • Contracted reimbursement
  • Prior authorization requirements
  • Physician credentialing and enrollment
  • Documentation and coding requirements
  • Implant, device, and supply costs
  • Billing and denial workflows

A newly covered procedure doesn’t automatically mean a profitable procedure.

The opportunity depends on whether the case is appropriate for your facility and whether your contracts and revenue cycle can support it.

Before You Add the Case, Know the Revenue

The 2026 ASC expansion could create meaningful opportunities for certain specialties.

But the goal shouldn’t simply be to perform more procedures.

The goal is to identify the right procedures, for the right physicians, under the right payer contracts, with the right revenue cycle processes in place.

Peregrine Healthcare helps ASCs evaluate the entire revenue picture, including managed care contracting, credentialing and enrollment, authorization and front-end processes, coding, billing, denial management, and collections.

No. Medicare's ASC Covered Procedures List does not determine a commercial payer's coverage or the terms of an ASC's payer contract. Facilities should verify payer-specific coverage, authorization requirements, and contracted reimbursement before adding new case volume.
