Supervision and Split/Shared Rules at a Glance
- Supervision levels (direct, general, and immediate) define how available the supervising provider must be during patient care.
- Split/shared services apply to facility-based E/M visits when both a physician and an NPP share responsibility for the encounter.
- The provider who performs the “substantive portion” (more than half the total time or majority of medical decision-making) should bill the visit.
- Virtual supervision continues to be allowed through December 31, 2025, under current CMS policy.
